Die von Willebrand-Faktor-spaltende Protease gehört zu einer Familie von Zink-haltigen Metalloproteasen. ADAMTS-13 steht für a desintegrin and metalloprotease with thrombospondin-1-like domains 13. ADAMTS-13 reguliert die Größe und damit indirekt die biologische Aktivität des von Willebrand-Faktors. Ein funktionaler Defekt oder Mangel dieses Enzyms führt zu ungewöhnlich großen von Willebrand-Faktor-Molekülen (UL-vWF) und dadurch zu erhöhten Plättchenaggregation.
Klinische Bedeutung
Durch die hohe Konzentration an supranormalem von Willebrand-Faktor kann es zu thrombotischen Mikroangiopathien kommen. Beispiele für solche thrombotischen Mikroangiopathien sind die thrombotisch-thrombozytopenische Purpura (TTP, Morbus Moscowiz), das hämolytisch-urämische Syndrom (HUS) sowie das HELLP-Syndrom. Diesen Krankheitsbildern liegt eine Störung des Blutflusses in Arteriolen und Kapillaren zugrunde. Nach heutigem Kenntnisstand ist für die TTP eine Störung der Funktion von ADAMTS-13 ausschlaggebend.
Die häufigste Form der idiopathischen TTP tritt als Autoimmunkrankheit ungeklärter Genese auf. Hierbei bilden sich Autoantikörper gegen ADAMTS-13. Demgegenüber steht die hereditäre Form (Upshaw-Shulman-Syndrom). Durch einen genetischen Defekt auf Chromosom 9 kann ADAMTS-13 nicht korrekt synthetisiert werden. Es sind mittlerweile 40 Mutationen bekannt. Weitere Auslöser für eine TTP können Schwangerschaft, Knochenmarkstransplantation, Medikamente oder Infektionskrankheiten sein.

- Differenzialdiagnostik zur Abklärung möglicher Ursachen von thrombotischen Mikroangiopathien.
